A vitamin B12 deficiency can go unnoticed for years until physical and neurological symptoms appear. 

The most common signs include extreme fatigue, pale skin, a swollen tongue, and, in advanced cases, nerve damage that can cause loss of balance, numbness in the extremities, and cognitive impairment.

Vitamin B12, also known as cobalamin, is crucial for the formation of red blood cells, proper functioning of the nervous system, and DNA synthesis. 

When its level is low, the body cannot produce blood cells efficiently, causing megaloblastic anemia. This condition affects both the body’s oxygenation and neurological health.

Early symptoms of B12 deficiency: warning signs

Who is at risk for vitamin B12 deficiency?

Vitamin B12 deficiency primarily affects older adults, people on strict vegan or vegetarian diets, patients with digestive disorders such as atrophic gastritis, Crohn’s disease, or celiac disease, and those who have undergone gastric or intestinal surgery. Additionally, certain medications such as proton pump inhibitors, H2 blockers, and metformin can interfere with B12 absorption.

If you experience any symptoms associated with vitamin B12 deficiency, it is essential to consult a healthcare professional. Diagnosis is confirmed through blood tests that evaluate B12 levels, hemoglobin, and other associated variables.

Treatment depends on the cause, but often includes high-dose oral supplements or intramuscular injections of vitamin B12. Symptoms may begin to improve within a week, although the most severe neurological effects may take up to three months to reverse, if they have not become permanent.

Complications of untreated B12 deficiency

Ignoring a B12 deficiency can lead to severe anemia, irreversible nerve damage, heart failure, and an increased risk of autoimmune diseases or certain types of gastric cancer. Therefore, timely action is essential.

Differences with folic acid deficiency
The symptoms of folic acid (vitamin B9) deficiency may resemble those of B12 deficiency, such as fatigue, paleness, or neurological disturbances. However, B12 deficiency develops more slowly and often includes jaundice and more severe neurological disorders. A proper diagnosis will help differentiate between the two.

To avoid deficiencies, adults should consume at least 2.4 micrograms of vitamin B12 daily, preferably through animal products, dairy products, fortified cereals, or dietary supplements. People at higher risk may require lifelong supplementation.
